Core responsibilities:
Assist patients with activities of daily living (ADLs): bathing, dressing, toileting, eating, mobility and personal hygiene.
Take and record basic observations: temperature, pulse, respiration, blood pressure, weight (according to local protocols).
Support with feeding, nutrition and fluid intake monitoring.
Help patients with transfers, positioning and use of mobility aids (following safe manual handling procedures).
Observe and report changes in physical/mental condition to the nurse in charge.
Assist with wound care dressing changes and basic clinical tasks only as delegated and within competency.
Maintain patient dignity, privacy and confidentiality at all times.
