Core duties / responsibilities
Assist with personal care: bathing, dressing, toileting, continence care, oral hygiene.
Help with mobility and transfers (use of hoists, wheelchairs) and fall prevention.
Take and record basic observations: temperature, pulse, respiratory rate, blood pressure, oxygen saturation, fluid intake/output.
Assist with feeding and ensuring special diets are followed.
Support nursing procedures (prepare trolley, clean equipment) under supervision.
Turn and reposition immobile patients to prevent pressure injuries.
Observe and report changes in a patient’s condition to the nurse in charge.
Maintain accurate, timely documentation (care charts, fluid charts, observation logs).
