Key responsibilities:
Accurately dispense prescription medications and compound preparations when required.
Review prescriptions for appropriateness, dose, allergies, drug interactions, duplicate therapy, and insurance/coverage issues.
Provide patient counseling on proper use, side effects, storage, adherence, and monitoring.
Perform medication therapy management (MTM), chronic disease management, and reconciliation at transitions of care.
Verify and supervise sterile and non-sterile compounding per USP and facility policies.
Collaborate with physicians, nurses, and other clinicians to optimize therapy and recommend alternatives.
Administer immunizations and point-of-care tests where permitted by law.
